Final Attendees

Please select your age group and the club must complete the form on behalf of all attendees, this should not be completed by individuals. This must be received 48 hours before your kick off date and time. Clubs will be invoiced for the attendance fee for the U11-16 finals. Attendees will be registered upon entrance for Test and Trace purposes. We will hold the data for 21 days after the final takes place for Test and Trace. Everyone should self-assess for COVID-19 symptoms before attending. If you are symptomatic or living in a household with possible or actual COVID-19 infection you must not participate or attend.
